Endoscopic Submucosal Dissection (ESD): Scarless Tumor Removal for the Stomach and Colon
What is ESD?
Endoscopic Submucosal Dissection (ESD) is an advanced, minimally invasive medical procedure used to remove tumors from the gastrointestinal tract (such as the stomach or colon) without the need for abdominal surgery. This high-tech technique allows specialists to remove early-stage or large tumors completely intact in a single piece (en bloc resection). This intact removal ensures highly accurate pathology results, allowing for the most precise and effective ongoing treatment plan. Because there are no surgical incisions on the abdomen, patients experience a much faster and more comfortable recovery.
Who is ESD for?
The ESD technique is highly suitable for patients with large precancerous lesions (tumors larger than 2 centimeters). It can be safely performed throughout the digestive tract, including the esophagus, stomach, small intestine, and colon. In Thailand, these lesions are most frequently treated in the colon.
Key Advantages of ESD Treatment
- Incision-Free: The tumor is removed entirely through an endoscope, leaving no external surgical scars.
- Complete Removal: Allows the tumor to be extracted perfectly intact in one single piece.
- Rapid Recovery: Requires a very short recuperation time compared to traditional open surgery.
The ESD Procedure
- General anesthesia is administered to ensure you are completely asleep and comfortable during the procedure.
- The specialist carefully inserts a flexible endoscope either through the mouth (for stomach lesions) or the rectum (for colon lesions) to reach the exact location of the tumor.
- Using micro-instruments, the doctor dissects and extracts the tumor through the same natural opening. The intact tissue is then sent to the laboratory for a comprehensive pathological analysis.
- The procedure typically takes between 2 to 8 hours, depending on the size, location, and number of tumors being removed.
Post-Operative Care
Initially, you will be required to fast (no food or drinks). Following your doctor's assessment, you will gradually transition from sipping water to a clear liquid diet, then to soft foods, and eventually back to your normal diet as your digestive system recovers.
Pre-Operative Preparation
- Stop taking antiplatelet or anticoagulant medications (blood thinners) for at least 7 days prior to the procedure, as strictly directed by your physician.
- Please inform your doctor about any underlying medical conditions and any history of drug allergies.
- Fasting (no food or drinks of any kind) is required for at least 6 hours before the procedure. For colon procedures, you must also take prescribed laxatives to completely cleanse your bowels according to the hospital's specific instructions.
Estimated Cost for the ESD Procedure
- Approximately 250,000 - 500,000 THB
