What is checked in a stool test?
1. Fecal Occult Blood (FOB)
This test is easy and inexpensive, gives results within 1 hour, and does not require fasting from food or water. However, it has limitations: its sensitivity for detecting pre-cancerous lesions is relatively low (<65%), and there is a risk of **False Negative** results (blood is present in the stool, but the test result is negative) in patients who are not currently bleeding.
2. Transferrin Test
Transferrin is a protein found in the blood. Detecting high levels of it in the stool indicates that there may be bleeding in the gastrointestinal tract.
3. Calprotectin Test
This is a blood protein excreted in the stool when inflammation occurs in the gastrointestinal tract. High levels indicate clear intestinal inflammation, commonly seen in patients with chronic inflammatory bowel disease, intestinal infections, or colorectal cancer.
4. Lactoferrin Test
This is a blood protein that, when found at high levels in the stool, can be used as an indicator of gastrointestinal inflammation, similar to Calprotectin.
Combining the FOB test with checking the levels of Transferrin, Calprotectin, and Lactoferrin (Inflammation Screening; **Stool QuadTest**) increases the sensitivity for detecting pre-cancerous polyps (67-76%). This allows for early treatment with minimally invasive endoscopic surgery and fast recovery. However, there is a risk of False Negative results in patients taking non-steroidal anti-inflammatory drugs (NSAIDs).
What diseases can be screened for?
Stomach ulcers, infectious colitis, diverticulitis, hemorrhoids, non-cancerous polyps and tumors, or malignant colorectal cancer, etc.
Who Should Get Screened?
- Individuals **aged 40 and older**.
- Those who experience frequent **bloating, constipation, or alternating constipation and diarrhea**, or have **blood/mucus in their stool**.
- Frequent **abdominal pain, cramping, discomfort, or griping pain**.
- Those with **chronic inflammatory bowel conditions**.
- Individuals noticing **thinner stool size**.
- Those with **unexplained weight loss or anemia**.
- Individuals with a **family history of polyps or colorectal cancer**.
- Those who wish to screen for gastrointestinal abnormalities should get checked **annually with their health check-up**.
Guidelines for Stool Sample Collection
- Do **not** let the stool sample come into contact with water or urine.
- The collected stool must be neither too hard nor too liquid.
- Avoid collecting stool during **menstruation** or if you have **bleeding hemorrhoids**.
- If you have **constipation or diarrhea**, postpone the stool collection.
- For at least **7 days before collecting** the stool, **stop taking NSAIDs** (Non-Steroidal Anti-Inflammatory Drugs)—the group of medications for pain, fever, and inflammation that irritate the digestive tract—such as Aspirin, Ibuprofen, Naproxen, and Indomethacin.
